Psilocybin: The leading regulatory path for PTSD.

In January 2026, the FDA accepted Compass Pathways' IND application for psilocybin in PTSD, enabling a Phase 2b/3 multicenter, randomized, double-blind trial.

A published Phase 2 open-label study (N=22) of single-dose 25 mg psilocybin in PTSD patients showed rapid and durable symptom improvement at 12 weeks, with favorable tolerability.

Psychedelics like psilocybin act on 5-HT2A serotonin receptors, temporarily disrupting rigid default-mode-network patterns and creating a window of neural flexibility in which trauma memories can be reprocessed.

N=26 combat soldiers. ~84% PTSD remission at end of retreat across both cohorts (~78% durability at 1-month follow-up in Cohort I). ~77% mean depression reduction, ~73% mean anxiety reduction.
